Cooligy offers a comprehensive employee

MEDICAL INSURANCE
Administered by Blue Cross, Cooligy offers two options:
--PPO (PREFERRED PROVIDER ORGANIZATION), $10 co-pay, 90/70 plan
--HMO (HEALTH MAINTENANCE ORGANZATION)
DENTAL INSURANCE
Under our plan, preventative dental care services are fully covered; basic
services are covered at 80% after deductible; major services are covered
at 50% after deductible, orthodontia services for children are covered at
50% with a $1500 lifetime maximum.
VISION CARE
PLAN
This plan covers eye exams, lenses, and frames. In-network services
offer increased benefits.
401(K) EMPLOYEE
SAVINGS PLAN
Cooligy offers employees the opportunity to save and invest
income on a before-tax basis through payroll deductions. You will be
eligible to allocate your pre-tax contribution to 17 investment options.
You may also roll-over monies from a previous 401(k) fund on a lump-sum
basis.
